1. Pageant Entry Form
2. Pageant Rules and Regulations Form (retain a copy for your reference)
3. Pageant Entry Fee - check payable to: Carroll Community Festival /
Memo Section: “Festival Baby Pageant” or "Festival Kid/Teen Pageant"

Submit via US Mail with payment - please ensure sufficient postage:
